Importance of an AFIX

AFIX (Assessment, Feedback, Incentive, eXchange) is a continuous quality improvement process for providers to learn about and to improve their immunization practices and rates. AFIX assessments look at whether your patients, 24-65 months old and/or 13-18 years old are up-to-date with recommended vaccines. During the feedback session you receive reports showing your immunization rates, we identify your strengths and opportunities for improvement.

MOBI/TIES

MOBI (Maximizing Office Based Immunizations) and TIES (Teen Immunization Education Sessions) are statewide provider immunization education and training programs developed for health-care providers. These continuing education programs offer techniques to improve immunization rates in the practice. They are a one-hour in-office continuing education program, and new for 2017 offers MOC Part II for physicians.

WIC - By Appointment Only.  Monday - Friday 8:00 AM - 4:30 PM
          Walk-In Newsletters:  Tuesday:  11:30 AM - 1:30 PM; Thursday:  1:30 PM - 3:30 PM

Pt. Care Walk-In Clinics Monday-Wednesday-Friday 9:00 AM – 10:00 AM and 1:00 - 4:00.
