Infectious Disease Reporting

FaxMachine2All reporting to the Wayne County Health Department's Infection Control Program is confidential. Faxes are sent to a secure fax and go directly to Infection Control. To report, please fax patient's demographics (name, date of birth, sex, address, and telephone number), diagnosis or suspected diagnosis, treatment, and provider information (name, telephone number, and complete street address). Please report to the health department of county residence.

After-Hours Reporting

Providers that need to report a Class A1 infectious disease after hours should call (330) 287-5700. The emergency dispatacher will contact the health department's representative.

Vital Statistics (birth and death certificates) issued Monday - Friday  8:00 AM - 4:00 PM
Environmental Health Monday - Friday 8:00 AM - 4:30 PM.  Closed for lunch from 12:00 - 1:15
WIC - By Appointment Only.  Monday - Friday 8:00 AM - 4:30 PM
          Walk-In Newsletters:  Tuesday:  11:30 AM - 1:30 PM; Thursday:  1:30 PM - 3:30 PM

Pt. Care Walk-In Clinics Monday-Wednesday-Friday 9:00 AM – 10:00 AM and 1:00 - 4:00.
