Vital Records - Death Records:

Certified death certificates are available for $19.00 per copy from the Wayne County Health Department for persons who died in Wayne County, Ohio after December, 1908.

Please note: Only death certificates for deaths that occurred in Wayne County, Ohio are available from our department. Death certificates for other counties must be obtained from those counties or from the Ohio Department of Health (see additional information below.)

DisclaimerPlease be aware that the local and state Vital Statistic Offices must comply with Federal regulations.  As federal and state laws change, older birth certificates may have to be replaced as certain federal agencies will begin to refuse to accept these older certificates.  Replacement certificates will be available at the prevailing rate for Certified Copies.
 
"Certified" death certificates are required for most legal purposes and are available for $19.00 per copy.

Uncertified death certificates are no longer available in Ohio as of July 1, 2003.

 Obtaining Vital Statistic Records Via Mail :

 
Alternatively, you may obtain certified death certificates by mail. If you would prefer to order a certified death certificate by mail, please list the following information along with a self-addressed, legal-size stamped envelope with payment and mail to Wayne County Health Department - Vital Statistics, 203 South Walnut Street, Wooster, Ohio 44691.

1. Requester's name, address, phone number.
2. Type of record (death) and number of certified copies desired.
3. Last name (at death), first name, and middle initial of the desired death certificate.
4. Date of death of individual for whom the birth certificate is being requested.
5. Place of death, if known, (Hospital name, home, etc.) including Ohio city/village/township information.
6. Parents names and, if known, the mothers maiden name.


Note: No death certificates will be mailed until payment is received - No out-of-state personal checks can be accepted and out of state payment should be made by a certified check or money order that is made out to the "Wayne County Health Department."
